On my regularly scheduled work date, I exceeded my first/second meal period break. I hereby certify that my employer provided me with the opportunity to take all rest breaks and meal periods on the workday and I am fully aware that my employer requires that I take a meal period of 1 hour before the fifth hour of work starts unless I work less than 6 hours on any day. I also understand that a second 30-minute meal period is required if I work beyond 10 hours unless my workday ends at 12 hours for that day and a meal period waiver for the first meal break was not required.
